Royal Rehab Ryde can provide a specific, short-term rehabilitation program in your home or community following hospitalisation or changes in your function due to an illness, injury or exacerbation of a chronic condition.

Home-Based Rehabilitation Programs

Royal Rehab provides a range of allied health therapies to clients in their homes after being discharged from hospital, with the aim to assist in transitioning back to home life.

 

Our home-based rehabilitation program encompasses the following key features:

 

  • Neurological Rehabilitation: Tailored for conditions like stroke, Parkinson’s Disease, and multiple sclerosis.
  • Orthopaedic Rehabilitation: Addressing needs arising from fractures, joint replacements, and amputations.
  • Reconditioning and Recovery: Specialised support for post-surgery recovery, hospital stay debilitation, and chronic condition exacerbation.
  • Comprehensive and Holistic Approach: Our team comprises occupational therapists, physiotherapists, speech pathologists, dietitians, social workers, allied health assistants, and a registered nurse.
  • Flexible Support: Access additional support services as needed, including neuropsychologists and rehabilitation medical specialists.
  • Collaborative Goal Setting: Our approach involves setting joint goals with you, your family, or those involved in your care.
  • Seamless Transition: Referrals to relevant services post-program and a discharge summary with goals provided to you and your general practitioner.

To be eligible for our home-based rehab program, you must meet the following criteria:

  • Reside within the Northern Sydney Local Health District.
  • Be 16 years or older and medically stable under the care of a general practitioner.
  • Have recently experienced injury, illness, surgery, or an exacerbation of a chronic condition.
  • Possess functional goals achievable within a six-week timeframe.
  • Have a safe home environment and necessary support services in place.
  • Consent to receiving the program at home, with agreement from your family or carer as needed.
  • Require a multidisciplinary program involving a minimum of two disciplines.

Referrals to the program can be made by your treating inpatient team or your general practitioner. Referrals from hospital should be made at least 48 hours prior to planned discharge date. To find out more call 02 9808 9687 or email cbrs@royalrehab.com.au.
